Actually this happened this last summer in Mountain Home Idaho, just 30 minutes away from myself. Anyways, the pilot took off rolled inverted and was supposed to loop, i think a figure eight type of move. He was reading his guages at ASL instead of AGL. There is cockpit footage at F16.net.

The paperwork indeed. The guy is now flying a desk.

He f*cked up with altitude above sea-level and altitude above ground, commenced a split-s too low and had to punch out. He was in display team, but is now permanently grounded.
