Fighting in the Dutch East Indies was primarily a British, Australian, New Zealand and Dutch affair. Compared to other campaigns, the US was minimally involved in the Dutch East Indies.

After the fall of Java in March 1942 the NEI were largely ignored as a major theatre of operations, with the exception of some operations in Dutch New Guinea such as the liberation of Hollandia. Later, in 1945, Tarakan and Balikpapan were liberated, these being major oil ports. However, these operations were unnecessary since by then Japan had been cut off from the NEI by (a) the liberation of the Philippines and (b) the destruction of Japan's merchant navy, without which the oil could not have been transported to Japan anyway.
The British Eastern Fleet did carry out several air strikes against Palembang and Soerabaja plus a few raids by surface forces such as the raid on Sabang, but these were of a minor nature compared to operations carried out elsewhere in the Pacific theatre.
Pior to the fall of Java, however, there WAS considerable U.S. involvement in the NEI.

MacArthur had nothing to do with Iwo or Okinawa or the other Central Pacific island battles. This was a totally different Theatre of War and run by Admiral Nimitz. In fact, it competed for supplies and strategic priority with MacArthur's South Pacific Theatre of War.
