(re)install the driver of the graphic card and make sure it's the latest version.
(GeForce GTS 450 drivers)
/EDIT
Also update your Monitor drivers. 11-02-2006 is OLD.

-----------------------------------------------------------------------------------

It looks like you are having some kind of Device Driver/hardware conflict on your PC.....

This file i.e. vgapnp.sys might be the cause. How old is your Monitor ? Have you properly installed/updated any VGA monitor drivers ?

I suspect there are some files corrupted, please run SFC to have a check. Go to Start---Search, and Type the following command, and then press ENTER:

sfc /scannow

The sfc /scannow command will scan all protected system files, and replace incorrect versions with correct Microsoft versions. Give me the results after scanning. Is your BIOS set to default ?

Maybe Windows did not find any integregation violations, I did with that list.
- Could not find driver software.
- Could not load driver software.
- Video hardware error.

Uninstall the graphic driver(s) found and re-install them all. Make sure all of them are up-to-date.

................

This is Not an issue with your Graphics card driver issue. It could be your Vga Monitor drivers, which are creating a conflict, as KingDaniel0 told above.

Because from your DXDIAG, it also clearly shows that you have the GPU drivers installed. (Driver Date/Size: 3/8/2011 03:43:00, 10084968 bytes).
