there should be someway you can either choose or change your specialised skill..i see heaps of head doctors and not
much of any other speciality, maybe you can add extra training or once you get pass lvl 10 you can chane your speciality..
instead of it being random

I like the idea of being able to pick your specialty, whether it's at the beginning, or later as you suggested.

I agree, it would be cool if we could respec/'Re-Train' at the cost of budget or possibly House Credit (Increased cost per respec). It would be a cool feature. Then again it would prove some instability in the game, say if everyone's always re-training you may find yourself with no Doctors of a certain Specialty. Also it could make getting the Panacea increasingly easy or difficult to get.

Neat idea. Use budget to pay for training, so you can choose what type of medicine you want to practice.
