occurred after finishing Zone 2 of the cemetery map, in the insurgent DLC. never had this problem before.

how to fix it?

this is odd. i got the error when i was doing a hand to hand only (ok, i did use the pistol to shoot the lights that the Ubisoft developers allowed me to shoot at).

but when i did a shoot-through game few minutes ago, everything's fine.

This is very famous bug of New Orleans level. I found it two years ago. If you want to avoid it, just shot at least one guy with mark and execute in the Zone 2! Or make something like this. By the way, this walkthrough by Andre202.
