I'm stuck on a case (non clinical) where I need to buy a tongue depressor for 2 house credit and I have only one left.

Do the clinic minigames and choose a patient to send to the pharmacy who has a tongue depressor. You don't need House Credits at all. If there is no patient in your clinic that shows a tongue depressor, one will eventually show up. Just keep trying.

So -- really the only way to play is to use real money to buy pretend money. That's kind of rotten. Too bad, because otherwise the game is fun.
