I think more minigames definitely need to be added, I enjoy the cell game but the matched pair one not so much.
I think there should also be more interactions with other characters such as House, perhaps he could give you mini-cases so that you can do something other than treating patients from the clinic and the main case.
It should be harder to earn the trophies since at the moment they're easy to get and so not very rewarding and I feel that you should be able to earn experience or money from completing them.
Being able to customize your character would be nice, atm there isn't a lot of choice.
Would also be nice to be able to interact with neighbours more, such as actually visiting their hospitals. Something like a morgue could be added.
