I know what you mean! I once made about 6000 cash in one day, thanks to the 2x bonus, and thought 'Oh well, this will last me ages now!', only to find that the next day, I was poor again! Especially on the last challenges, when you're required to spend 1000+ cash in between challenges, and need about 3 favors and 20+ meds!
I don't know if it's just me either, but has anyone noticed that when you play the Blood machine on a general clinic patients, you're inundated with red blood cells, and have absolutely no chance of getting anything else, because the red cells just fly about everywhere! I don't know if this is a bug or not, because when you go to play the Blood machine as part of the case, the number of cells is massively reduced, so you actually have a chance of playing the game. When I play on the cases, I can get 20-25 in a row, no mistakes, but when I play the machine for the clinic patients, I'm lucky to get 10, and usually have many more errors. Is this just me?

I personally have no intention of buying house bucks, I'm lucky to get through the month with enough food, I'm certainly not going to spend on a silly game. You at U play need to take notice, when the game stops being fun because I can't care for my patients, then I move on to another game, because like many people I have no extra cash to spare to spend on a game.
There is nothing you have to do in this game that requires you to spend any money. If you do, it goes faster, that's all.

Yeah, we all hear that argument and yes, technically you are right. I also know that ubsoft is not a charity and doing this out of the goodness of their hearts, nor do I expect them to be. The job of a free player is to shill this game to their friends on the hopes that some of the friends will be willing to plunk down real cash.
So, the trick is to keep the game amusing enough for the free players that they keep playing it, that they keep encouraging their friends to play. You need the "salesmen" to keep selling the game. Unless you want to take the attitude of, "Well, who cares if people stop playing, we'll just get new players!" Then you wind up like Zynga who realized that yeah, it's possible to run out of new players.
Do I think Ubsoft should just give free players everything? No. But they need to come up with a balance that will let even the free players progress fast enough so that they feel they are getting game play. As it is now, I feel like I'm spending a long time doing boring things, just to earn enough to be able to get about a minute of "real" game play. How long is that going to keep me enthralled with this game? Not long.
I spent money once for Housepoints, because I was new and I wanted to really give the game a shot. I won't do it again until the game proves that it's fun to play, even if I don't have 25-50 dollars to shove into it every week. I've never been afraid to spend some of my "entertainment" budget on these games, but since entertainment budget is something that isn't set in stone, I only want to spend real money on games that can afford to keep me amused, even those weeks/months when I can't toss money into them.
