Can someone enlighten me (and others) as to the differences between those 3 modes?

I have finished the game in normal and switch to hard but I find no differences in the game play. There's probably the added baddy here and there but nothing else.

Alledgedly the enemy AI have a mind of their own when playing in hard mode or that they would change the way they appear (spawn or respawn) when you re-start a mission but I haven't seen that.

Is that a glitch on my pc only or is everybody in the same boat I am.

P.S.: I really would like replies that are relevant to the question and not tangent replies of another game or irrelevant to this.
