I believe you have to garner enough renown before the choice to get an emblem will be presented in the after-patrol u-boat screens listed under "specials" - I can't remember how much renown you need, but it took me 2 or 3 patrols before I could get one, IIRC.

There is also a way to make your own emblems and add them to the list of ones the game will let you pick - I'm using a nice picture of my former Ithorian toon in red armor from Star Wars Galaxies right now, but am about to make one of my dog's head - that should be a little more historically appropriate I guess

IRRC, you just need to make your picture a 64x64 pixel .tga file in 24-bit colors, then put a copy in the SilentHunterIII\data\emblems\subemblems folder along with the rest of the ones there - then in game yours will show up on the list and you can choose it whenever you are given the choice to get an emblem (once you get one, you can change emblems after any mission)
