It's a stealth-o-meter like the one in the game Thief, it shows you how good or bad the enemy can detect you, green => ok, red => the enemy got you.
Thanks. Seems kind of unrealistic(probably can turn off) but better than a health meter.
Why unrealistic? is that you cannot hear yourself if you are making noise? Actually, modern subs use their towed sonar to detect if they make sound or not, as a drill.
I supose it doesn't says "the enemy got you" but that it says how much noise is making your sub at that moment. For examble: ahead flank, red. Shooting torpedoes, yellow. Silent Running, green.
Alsop, it gets more red if you are navigating with the periscope, as in SH3, ships will sight your periscope, and they will sight it for sure if you have it raised all the time.
Look at the in-game footage video and analize it, man! You'll see this things.

That should be how it should work. It would be really unrealistic if it told you exactly when the enemy saw you. I think it must be a gauge of how visible your sub is, and not necessarily if the enemy has spotted you or not.
