You may have manual loading selected, in which case you have to go to the torpedo management page and move a torp from reserve to a tube...or turn off manual loading. It's a bug. It happens sometimes when you save and exit with empty torpedo tubes. Always reload all tubes before exiting the game.

I'm a big fan of manual torpedo loading. I customize my loadout at base; mostly steam powered eels up front, but at least one electric fish in a bow tube at all times and one in stern reserve, for those occasions when I may encounter a warship. The electric fish are slower, but since they don't leave a visible wake it is highly unlikely the enemy will see it coming and dodge it at the last second.

Also, if you are loading the game through SH3 Commander, check your settings to see if you have Malfunctions and Sabotage enabled. If so, all sorts of things can stop working from time to time.
