We tried to mix the chemicals but we don't know where to find the notes that tell us which colors to add at what time. Where are the notes.

Once you have accessed the server computers you need to check your notes on the OPSAT as this is where you'll find the necessary codes and blood type.

You have to access the computers in the server room downstairs to get the proper mix sequence. Each player will have a different series of letters to use and you will also find out which blood type it is based on (A, B, or O).

In the mini-lab you will need one player to grab the required blood type and the other player can grab either of the two left.

Once you get to the mixing chamber, you will have to enter the sequence at the same time and in the proper order. For instance, if player 1 has "E, A, F, B" and player 2 has "A, G, B, F" (depending on the level of difficulty you may have more letters than this), you must select the proper mixing section by letter like this:

P1 = E, P2 = A
P1 = A, P2 = G
P1 = F, P2 = B
P1 = B, P2 = F

You have a limited time to do this once you start, IIRC, so don't take too long between each selection. If you mess up, it will explode and you get mission over so have a save point just before starting.
