I had the same problem. I broke my harddrive by accident and had to replace it.When I reinstalled my game on my new harddrive the game worked perfectly ever since, none of the irritating empty world stuff.

Your hard-drive could be the cause to the problem.

I would say more to installing every thing fresh and updated was the better solution.

Files can get so corrupted after a while.
