Everything I've ever read about it says the twin engined P-40 was a mock-up only and never flew.

Mock up it was Berg, never had engines fitted from what I've read.

As for the latest question, Boulton-Paul P.92/2 built by Heston Aircraft, a 1/2 scale proof of concept, which never flew before the project was cancelled.

Clue: It was in front line service from the Spanish Civil War till 1941 and was still in service until 1943.
