why is it happening and what's the solution?

Actually there are many causes of this error e.g. the latency of the memory i.e. RAM, as well as the 'HW Transform & Lighting' video option. I presume this is for FAR CRY, and are you trying to run the game editor ?

What are your system specs, and what in-game settings are you using to run the game ??

Give the following method a try.

Go to Start > Run. Then type 'regedit' and click on Ok. Navigate to the path HKEY_LOCAL_MACHINE\SOFTWARE\Crytek\FarCry\PatchVersion.

Change the value what it says from 10040 to 10030, and then Reboot your PC. After that you can install the 1.4 cumulative patch. Hopefully this should solve your problem.
