How do I transfer UPlay to another gamertag?

I had to create a new gamertag on Xbox Live. I didn't realize my UPlay account was linked to the other one and now I started playing Project Legacy on Facebook and it's also registered to my old tag. How do I fix this? I don't wanna miss out on the points I can get from PL and I've already started playing Brotherhood on this new gamertag. Please help :/
