Achievement Glitch?

I have finished the game on Hard, but I noticed that for some reason I am still missing the achievements for missions 12 and 14 through 19. I have the campaign challenges and I have replayed several of the levels multiple times trying to get the achievements but with no luck.

Anyone else had this problem?

Yes it has happened to me while playing co-op. The host received the achievement but i did not. I have played the mission over solo and as host but have had no luck. I hope a patch will come out so I can satisfy my addiction for these mere achievements.
