You must create your "all.ini" file with notepad. Do it this way :

-create a new txt with notepad,
-instead of "new document.txt", rename it "all.ini", confirm the change,
-now, open it and write this :

[list]
campaign's name (for example fighter)

The name of the campaign must absolutely be the name of the folder conmtaining the campaign's missions. Have making a US folder is right. The medals must be in tga format and renamed award0 to award7 to work, but open (for example) the german campaign folder to understand what I mean. Here, you will also see some other differents files, especially some called award0.mat (do know about the .mat because I am at work, here...) to award7.mat with the award0.tga to award7.tga. As you have changed your medals name, do not copy the tga from the german folder, but only the other award files.

Also, you need a background image... you can for example take in the US single missions folder. It is 2 files : background.tga (or bmp ???) and background.mat. Copy them in your campaign folder. And finally you need the icon.tga !!! If my memory is good, there is one on www.il2skins.com.. (http://www.il2skins.com..). but most of all, the modt simple to have a US campaign folder is to download the excellent one that Extreme One has made !!! You fdind it at www.mudmovers.com (http://www.mudmovers.com). If you do so, you will not have to do all that I explained before... of course, if you find the medals you have better than the ones included in Extreme One's pack, save yours before...

The all.ini was only neccessary in the old IL-2 days. FB is more intelligent and will find the campaign folders without your help. So you can forget about the all.ini in FB.

Have you saved your campaign in a folder "mission/campaign/us/AVG/"?

If this is the case then most probably you didn't add a "info.properties" file to that folder. This is another text file that contains the text that is shown when you choose the campaign from the career menu. It looks like this:

Name YourCampaignName
Description Blabla first line\n Blabla second line \n\n Blabla new paragraph
